Healthy Columbia Willamette Collaborative
The Healthy Columbia Willamette Collaborative (HCWC) is a partnership between hospital systems, local county health departments, and coordinated care organizations in Clark County, Clackamas County, Multnomah County, and Washington County, that have come together to fund one regional Community Health Needs Assessment (CHNA). By jointly supporting a single CHNA, these partners are able to maximize their resources, avoid duplication of efforts and overburdening the community, and identify regional trends and opportunities for programs and services. It is important for Clackamas County because it helps deepens community partnerships, centers and emphasizes community wisdom, tells stories reflective of lived experiences, and informs and influences those important decisions that are being made on behalf of the community. Organizations that rely on the CHNA includes hospitals, public health related organization, community-based ones, etc., in order to improve the health of a community.
